Legal work status

Before an individual may start working, they must submit satisfactory proof of legal authority to be employed in the united states and Connecticut. This includes producing the information necessary for completion of an I-9 form. Failure to produce satisfactory proof of legal status to work in the United States and Connecticut will be a basis for denying employment or immediate dismissal if documentation is fake or expired.

I hereby understand in consideration of the fact that Euro-American Homecare, 1436 Berlin Turnpike, Suite 1A, Berlin, CT 06037 (the “Company”) will take photographs of me for inclusion in my caregiver profile and for other promotional purposes, hereby grant permission for the Company to reproduce the photographs taken of me, and to prepare and distribute a caregiver profile about me, for the purpose of publication, promotion, marketing, or advertising, in any manner or in any medium, for Company purposes.

I understand that nothing in this employment application or in statements made in an interview are intended to create a promise or contract of employment with Euro-American Homecare, LLC. I understand that employment with Euro-American Homecare, LLC is at-will and as a per day employee. I also agree that, as a per day employee-at-will, Euro-American Homecare, LLC will have the right to terminate my employment without notice and without cause at any time or simply not use my services. I will also have the right to terminate my employment without notice and without cause at any time. I certify to the best of my knowledge that the information provided in this application is true. I understand that any intentional misrepresentation of this information or intentional omission of requested information is likely to lead to a revocation of any employment offer or dismissal, even if discovered years later. I also authorize Euro-American Homecare, LLC to contact my former employers to investigate my work history and verify my qualifications. if I am a successful applicant, before Euro-American Homecare, LLC can consider hiring me or making me an offer of employment, I agree to submit to a comprehensive background check and will complete and sign a form that contains questions as to whether I was convicted, plead no contest or guilty to a crime involving violence or dishonesty or was subject to any decision imposing disciplinary action by the department of public health or the department of consumer protection.
